Step 1
Print and complete the Lease Application Form. (Word format - 184k)
Note: To expedite the application process, please have copies of the Company's two most recent Federal income tax returns available for reference.
Step 2
Gather additional documentation required to complete the lease approval process ("Lease Package"). The complete Lease Package must include:
For lease applications up to $250,000:
For lease applications from $250,001 to $500,000, you must also provide:
- (2) years of fiscal year-end audited or reviewed Financial Statements* (e.g., income statements, balance sheets, cash-flow statements) for the Company.
- Completed, signed and currently dated Personal Financial Statements for all Principals owning 20% or more of the Company.
- Latest year Personal Tax Returns for all Principals owning 20% or more of the Company.
- Business Bank Reference Information (Bank Name, Account Number, Contact Person, Telephone Number).
- Trade references (2).
*If the Financial Statements are compilations, provide two (2) years of Business Tax Returns.
For lease applications from $500,001 and above, you must also provide:
- Three (3) years of fiscal year-end audited or reviewed Financial Statements* (e.g., income statements, balance sheets, cash-flow statements) for the Company.
*If the Financial Statements are compilations, provide three (3) years of Business Tax Returns.
Incomplete packages cannot be processed.
Step 3
Forward the complete lease package via regular mail, overnight mail or fax to:

Upon receipt and review, Manitowoc Finance will notify you regarding whether the Company has been approved for an equipment lease.
